LPN Job Opportunity at Sky View Rehabilitation and Health Care CenterOur 192-bed Skilled Nursing Facility in Croton-on-Hudson, NY has an opening for LPNs.Job SummarySky View serves as a bridge between hospital and home, providing Post Acute Rehabilitation or Long-Term Care in a luxurious and hotel-like environment.Our team of skilled therapists creates individualized therapy plans to help residents reach their highest level of functioning as soon as possible.Responsibilities

• Provide daily support to unit leadership and respond to resident/family needs within the LPN scope of practice.
• Use the nursing process to set priorities and deliver quality direct resident care.
• Perform standard nursing treatments and medications, observing residents for desired or unwanted effects.
• Communicate with the care team to revise/update the Nursing Care Plan.
• Monitor and document vitals, treatments, and residents' responses.
• Prepare and oversee the delivery of care for CNA assignments and activities.Requirements
  • Graduate of an approved school of Nursing.
  • Licensed and registered in good standing in New York as an LPN.
  • Dedicated to providing clinically sound and compassionate care.
  • Must possess a spirit of cooperation, caring, and enthusiasm to create a conducive rehabilitation environment.
